System design is fundamentally about incentives. There is no perfect health system – in NHS-style systems, doctors are not financially incentivised to over-treat and surgery rates are generally lower than in private systems. The ideal health system is probably somewhere between the two extremes.
Whether care is delivered at home, in the community, or in hospital is shaped as much by payment flows and contractual arrangements as by clinical priorities. When incentives are misaligned, costs rise and patients remain in higher-intensity settings for longer than necessary.
